EVALUATION
- Initial discussion and overview of situation.
- Nursing Assessment of medical history and current medical needs.
- Living Situation Assessment and its limitations and what the living arrangement desires of the client are.
- Functional Level Assessment of every day living needs and what assistance may be required.
- Financial overview that will factor into decision making.
WRITTEN EVALUATION
- A written compilation of above information with suggested plan.
ADDITIONAL
- Carrying out plan of care.

You want a referral to an Assisted Living or Independent Living Facility.
EVALUATION
- Initial discussion and overview of situation.
- Assisted Living evaluation to determine which facility would best suit client's needs, i.e. - activities offered, size of facility, location. They vary greatly. I have done extensive evaluations on each one including reviews of regulatory inspection reports to ensure they are in compliance with regulations. I do not take referral fees from facilities and, therefore, remain totally objective and your advocate.
- Nursing Assessment with medical history and current medical and psychosocial needs.
- Financial overview that will factor into decision.

- Written compilation of above information with suggested home care or home health providers. Home care and Home Health companies vary greatly and I refer only to ones which I have personal knowledge of the quality of care including regulatory compliance.
