You want to make appropriate living arrangements for
yourself or a loved one:
EVALUATION
- Initial discussion and overview of situation.
- Nursing Assessment of medical history and current medical
needs.
- Living Situation Assessment and its limitations and what the
living arrangement desires of the client are.
- Functional Level Assessment of every day living needs and
what assistance may be required.
- Financial overview that will factor into decision making.
WRITTEN EVALUATION
- A written compilation of above information with suggested
plan.
ADDITIONAL
- Carrying out plan of care.
You want a referral to an Assisted Living or Independent
Living Facility:
EVALUATION
- Initial discussion and overview of situation.
- Assisted Living evaluation to determine which facility would
best suit client's needs, i.e. - activities offered, size of facility,
location. They vary greatly. I have done extensive evaluations on each one
including reviews of regulatory inspection reports to ensure they are in
compliance with regulations. I do not take referral fees from facilities and,
therefore, remain totally objective and your advocate.
- Nursing Assessment with medical history and current medical
and psychosocial needs.
- Financial overview that will factor into decision.
WRITTEN EVALUATION
- A written compilation of above information with recommendations.
ADDITIONAL
- Discussion with appropriate facilities of client's needs.
- Visits to facility with client.
- Ongoing oversight of client while in facility.
You want assistance in the home:
EVALUATION
- Initial discussion and overview of situation.
- Functional Level Assessment of every day living needs and
assistance required.
- Financial overview that will factor into decision.
WRITTEN EVALUATION
- Written compilation of above information with suggested home
care or home health providers. Home care and Home Health companies vary greatly
and I refer only to ones which I have personal knowledge of the quality of care
including regulatory compliance.
ADDITIONAL
- Ongoing oversight of client to assure home care or home
health is provided appropriately.
You live far away from a loved one or you are very busy and
want to be sure things are going all right:
EVALUATION
- Initial discussion and overview of situation.
- Assessment of current situation to include Geriatric Care
Management psychosocial assessment, Functional Level, and needs.
WRITTEN EVALUATION
- Written compilation of above information and recommendations
for ongoing oversight.
ADDITIONAL
- Implementation of plan and ongoing oversight.
You are not sure of medical treatment or symptoms are not
being managed:
EVALUATION
- Initial discussion and overview of situation.
- Complete Nursing Assessment of medical history and needs.
- Medication Profile Assessment and Symptom Analysis.
- Review of physician’s medical records.
- Discussion of relationship with physician which may include
visiting doctor with the client.
WRITTEN EVALUATION
- Written compilation of above with recommendations.
